For the muscle aches some people report while taking a statin, trials of CoQ10 point in different directions, and the pooled analyses of those trials disagree with each other. Three of the systematic reviews drew on overlapping sets of small randomized trials. A 2018 review of 12 trials found lower muscle pain with CoQ10, a difference of 1.6 points on the pain scales used. A 2020 review of 7 trials found no clear difference from placebo, and no difference in how many people stayed on their statin. A 2025 review of 7 trials found a smaller reduction, just under 1 point, with a confidence interval that reached almost to zero.
One trial confirmed under blinded conditions that the statin was causing symptoms before testing CoQ10, and it found no difference from placebo. It first checked, under double-blind conditions, which participants developed muscle pain on a statin but not on a placebo, then gave those 41 people either CoQ10 or a placebo alongside the statin for 8 weeks. Pain rose with the statin to a similar degree in both groups. In that trial, 36 percent of people with a history of statin muscle pain developed it again under blinded conditions. Separately, a pooled analysis of 19 large double-blind statin trials found that in the first year, about 1 in 15 reports of muscle pain or weakness among people taking a statin was due to the statin itself.
The CoQ10 trials are small and short. In the two most recent reviews they enrolled 35 to 76 people each, ran for 30 to 90 days, and used daily doses from 100 to 600 mg. Most enrolled people who reported muscle symptoms on a statin without confirming that the statin was the cause. In blinded statin trials, most muscle symptoms occur at nearly the same rate on placebo. One of the seven trials in the 2025 review compared CoQ10 plus omega-3 against omega-3 alone rather than against a placebo. One trial measured tiredness and weakness rather than pain. Among 60 adults aged 65 to 80 who reported statin-related fatigue, 60 mg a day of CoQ10 for 8 weeks, in a formulation made to be absorbed more readily, was followed by lower self-rated fatigue and better grip strength and movement test scores than placebo. That trial did not confirm that the statin caused the fatigue, the supplement's maker supplied the product, and no repeat of it has been published in full.
CoQ10 is paired with statins for a biochemical reason. Statins block an early step in the pathway the body uses to make cholesterol, and CoQ10 is made along the same pathway. A pooled analysis of 12 randomized trials found that statins lower CoQ10 in the blood, and in the trials supplements raised it. Whether that drop reaches muscle or produces symptoms is less settled. A study that took muscle samples found similar muscle CoQ10 levels in statin users with muscle pain, statin users without it, and untreated people with high cholesterol. In the confirmed-symptom trial, blood CoQ10 rose about fourfold on the supplement and pain did not change. A 2026 genetic study of about 38,000 statin users found muscle symptoms recorded more often in people carrying a gene variant tied to lower levels of an enzyme the body uses to make CoQ10, about 4.0 percent of those with two copies against 2.4 percent of those with none. That connects the body's own CoQ10 production to symptoms. It did not test whether taking CoQ10 changes them.
UK national guidance on statins, from NICE, advises doctors not to offer CoQ10 to help people stay on statin treatment. For people who report side effects on a high-intensity statin, the options it lists are stopping and restarting once symptoms resolve, switching to another statin, lowering the dose, or moving to a lower-intensity statin.
On safety, CoQ10 has few reported side effects beyond mild digestive upset and insomnia, and the NIH liver injury database rates it an unlikely cause of liver injury. It may interact with the blood thinner warfarin and with insulin. Statins are prescribed to lower cardiovascular risk.
